Dr. Paul Clark, CMIO of Concord Hospital, will facilitate a discussion about how a predictive clinical workflow platform aided with logistical planning and physician burnout during the COVID-19 pandemic and allowed his organization and field hospitals to feel prepared. This platform is integrated with all major EHRs, including Epic and Cerner, through the use of FHIR API enabled integration. The innovative abstraction layer is supporting Concord through the pandemic with real-time diagnostic result alerts and visibility of all patients being tested or confirmed COVID-19 by locating, attending physician and isolation status. Concord hospital is tracking every potential Coronavirus patient from admissions through discharge with this platform, reducing the average time physicians are required to spend in the EHR chart. It is crucial to prioritize physician well-being during the pandemic and this product was able to simplify the logistical tasks required to prepare for COVID-19 while simultaneously giving time back to doctors.

Just prior to COVID-19 hitting the State of Maine, Northern Light Health (NLH) completed a pilot project of a unit level dashboard called CareVIew and the development, testing, and launch of an enterprise level Command Center Dashboard. The real-time data provided by these tools quickly allowed us to isolate our COVID-19 patients and align COVID and non- COVID care areas to ensure both patient and staff safety. The foundational people and process changes of managing patient throughput and centralizing operations supported the rapid transition required by this pandemic. The use of real-time data during the pandemic positioned NLH to manage volumes across seven-member organization hospitals. Throughout each day and during both system and local Incident Command meetings, the use of the Command Center dashboard allowed leaders to maintain situational awareness for all areas of care. The information displayed on each board is fed from foundational Cerner solutions including the EHR and Capacity Management and updated in real time. Additionally, predicative analytics display anticipated volumes, supporting a proactive approach to staffing and patient placement transforming organizational management across the system. As NLH emerges from COVID, this technology will be expanded to advance digital transformation and support the advancement of innovation across the enterprise. This presentation offers insight into the roll-out of each dashboard, the integration with other technologies, lessons learned, and the importance of situation awareness.

Admission to a hospital is a stressful time for any patient however is especially challenging for behavioral health patients who often present in crisis. As such, it is incumbent upon the admitting nurse to ask only those questions most critical at the point of intake to inform the immediate plan of care and keep the patient safe. Northern Light Acadia Hospital, Northern Light Health’s freestanding Behavioral Health facility, partnered with Cerner in the Behavioral Health (BH) Admission History Essential Clinical Dataset Collaborative to streamline the intake process of adult behavioral health patients. The Essential Clinical Dataset (ECD) defines the data elements for specific workflows within the Electronic Health Record (EHR) that are essential to the patient story. The BH Admission history provides a streamlined standardized interview and assessment process, focusing on the patient data required for clinical decision making so the care team may provide quality care and promote positive patient outcomes. Applying the EDC framework to the behavioral health admission process enhanced workflow efficiency within the EHR, improved staff satisfaction with the documentation process, and increased time at the bedside as demonstrated by the project’s key performance indicators. For example, the total number of questions was reduced by 39% while the number of sections was reduced by 62%. Most notably however was the 60% reduction in average clicks or 200 clicks per patient/admission history and overall reduction in documentation time by over 10 minutes per patient translating to an annual savings of 223 hours. This presentation will share Northern Light Health Acadia Hospital’s success in decreasing nursing documentation burden while supporting an efficient, meaningful, and safe admission intake for adult behavioral health patients.
