This session will detail the practical aspects of system wide transformation to team-based care at Bellin Health. Learn how this transformation achieves the three wins: a win for the patient, a win for the care team, and a win for the system. Objectives include:
- Understanding the three components of Bellin Health’s model of advanced team-based care
- Learning how the roles of team members are transformed to empower them to work at the top of their skill sets as well as understand the training needed
- Determining the infrastructures needed to be successful
- Understanding how team-based care can help your practice achieve the Triple Aim and improve the well-being of physicians and staff
Presenter Details
Kathy Kerscher has been with Bellin Health for 19 years. In that time, Kathy has served in several operational leadership rolls in ambulatory care. Currently, Kathy is the VP of Primary care and Administrative Leader of Team Based Care Redesign. As the administrative leader of Team-Based Care redesign, she is responsible for leading the redesign across Bellin organization to achieve population health goals which includes three win’s – win for the patient, win for the care team, and win for the system. Kathy co-facilitated several Bellin Health Team-Based Care Training Camp Conferences. As the VP of Primary care, Kathy is responsible for the strategies and success of 30 primary care clinics, 4 fastcare clinics, 5 Urgent Care Clinics and 170 employer clinics with on-site services.
Kathy received her MBA from Lakeland College.
